| Filing item | What to review |

| Principal office | Provide the business information requested by the form |
| Management | Identify the management structure and authorized information |

| Filing fee | $125 |
Before submitting
Confirm the name, registered office arrangement, principal information, management details, organizer information, and effective date. Save the completed filing and payment confirmation.
What the Certificate does not do
The Certificate forms the legal entity, but it does not:
- Create an EIN
- Write the operating agreement
- Open a bank account
- Register Pennsylvania tax accounts
- Register employees
- Obtain local or professional licenses
- File the annual report
After acceptance
Save the accepted Certificate, create the operating agreement, apply for an EIN when needed, and review Pennsylvania tax and licensing requirements. Add the first annual report to the following year’s January 1 through September 30 filing window.
Practical note
The approved Articles or Certificate creates the LLC record, but it does not finish the business setup. After approval, use the same legal name and address consistently on the EIN application, bank account, state tax accounts, licenses, and operating agreement. If the state record has a typo or old address, fix it before building more registrations on top of it.
